Nameer
Haider, MD, FAAPMR, AQPM, DABPM
Dr. Nameer Haider has been practicing Interventional Pain Management in New York State for over 10 years. Dr. Haider is a Diplomate of the American Board of Physical Medicine & Rehabilitation with a subspecialty Board Certification in Pain Medicine by the American Board of Anesthesiology. He also is a Diplomate of the American Board of Pain Medicine.
Dr. Haider graduated with Honors from King Edward Medical University, and then completed a rotating Surgical Internship at the East Carolina University School of Medicine in Greenville, North Carolina. He completed a Residency in Physical Medicine & Rehabilitation at the Northshore University/Long Island Jewish Medical Center in New York. He also served as Chief Resident at the Albert Einstein College of Medicine LIJ Campus. Dr. Haider then completed a Fellowship in Interventional Pain Management at the Spinal & Skeletal Pain Institute in New York.
He has been affiliated with several prestigious hospitals accross the United States. He currently serves as the Chairman of Physical Medicine & Rehabilitation at Faxton-St. Lukes Healthcare in New York, and is the President of the New York State Society of Interventional Pain Physicians. He is also the Fellowship Training Director for Interventional Pain Management at the Spinal & Skeletal Pain Institute.
Dr. Haider is a member of The North American Spine Society, American Academy of Pain Medicine, American Society of Interventional Pain Physicians, World Institute of Pain, International Spnal Injection Society, American Medical Association, American Academy of Physical Medicine & Rehabilitation, Association of Academic Physiatrists, Physiatric Association of Spine, Sports, and Occupational Rehabilitation, and the Medical Society of the State of New York. He is also serving as the Vice President of the Oneida County Medical Society.
Dr. Haider is actively involved with organizing and presenting academic seminars on behalf of the Minimally Invasive Spine Institute. He has traveled extensively throughout the United States and Europe providing patient and physician education. Dr. Haider has also presented research at many international surgical conferences andhis abstrats have appeared in leading publications. Dr. Haider is licensed to practice medicine and surgery in 5 states, and also holds clinical privileges at seven hospitals.
Dr. Haider is Co-Founder of Minimally Invasive Spine Institute, with offices in New York, Washington DC, and Northern Virginia. Dr. Haider uses cuting edge technology in the treatment of Spinal Pain. His area of expertise is in minimally invasive therapies for the treatment of Pain of Spinal and Skeletal origin. For precise localization and added safety, his procedures are preformed under live X-rat fluoroscopic guidance. Treatments provided include Foraminotomy, Percutaneous and Endoscopic Discectomy, Vertebroplasty/Kyphoplasty, Radiofrequency Ablation Rhizotomy, Surgical implantation of Spinal Cord Stimulators and Implantable Intrathecal Pumps, Spinal Fusion alternatives, and Minimally Invasive Spinal Fusion surgeries.

Stephen Wade, MD., FAAPMR, DABPM
Dr. Wade was born in Albany, New York. He graduated from high school in Brick, New Jersey and received his Bachelor of Arts degree from New Jersey City University in Jersey City, New Jersey. Dr. Wade then received a post-bachelor's degree in Nuclear Medicine Technology from the Institute of Allied Medical Professions in New York City.
Dr. Wade attended medical school at Our Lady of Fatima College of Medicine where he was freshman and sophomore class vice-president and did clinical rotations at Harvard University and The University of Medicine and Dentistry in New Jersey. He completed a one year intership in general surgery at Our Lady of Mercy Medical Center/Lincoln Hospital in the Bronx, New York.
Dr. Wade completed residency in the specialty field of Physical Medicine and Rehabilitation at Kingsbrook Jewish medical Center in Brooklyn, New York. During his residency, Dr. Wade presented a poster at the 2007 International Society of Physical Medicine and Rehabilitation in Seoul, Korea. He was also a Committe of Interns and Residents delegate and Chief Resident.
Dr. Wade is currently a member of AAPMR, ASIS, PASSOR, and AMA.
Dr. Wade currently resides in Utica, New York. in his spare time he enjoys classical music, running, cooking, traveling, and spending time with his friends and family.

Denny
J. Battista, DO, FAAPMR, AQPM, CIME
Dr. Battista received his Bachelor of Science degree at St. Bonaventure
University and graduated from the New York College of Osteopathic
Medicine in 1994. He completed a one year fellowship in Osteopathic
Manipulative Medicine and then a one year rotation in Internal Medicine
at Long Beach Memorial Hospital. He completed a residency in the
specialty field of Physical Medicine and Rehabilitation at Nassau
County Medical Center in 1998. He became Board Certified a year later in Phyical Medicine and Rehabilitation and is now a Fellow of the American Academy of Physical Medicine and Rehabilitation. In July of 2000 he became dual boarded by becoming a Board Certified Independent Medical Examiner.
After working for 5 years as an attending Physiatrist he developed a special interest in pain medicine. In 2004 he became triple boarded as he achieved Board Certification in Pain Medicine through the American Board of Physical Medicine and Rehabilitation and the American Board of Anesthesia.
Since then he has achieved certification in numerous fluroscopically guided interventional spine procedures and actively performs these procedures at the Clinton Office.
In January of 2005 he accepted the position of Medical Director of Faxton-St. Lukes Rehabilitation services and actively runs the CARF accredited acute inpatient rehabilitation unit with his colleagues.
in addition to providing quality comprehensive pain management and rehabilitation services, he also performs Electodiagnostic and EMG studies, Botox intervention, and Osteopathic Manipulation.
Lastly,
he resides with his wife Jennifer, their son Louis, and daughter Alexa, in New Hartford, NY.

Debra
Dermady RN, MS, ANP-BC
Debra Dermady is a Masters prepared Adult Nurse Practitioner who
graduated from SUNYIT Utica/Rome in 2001. She has been a registered
nurse for over 19 years, graduating from Mohawk Valley Community
College in 1989. She received her BS in nursing from SUNYIT in 1996.
She
was employed by Faxton - St. Lukes Health Care for 10 years working
in a variety of positions from a staff/charge nurse on a medical
surgical oncology unit to her final position as the charge nurse
on the acute rehab unit at Allen Calder.
After
graduation from SUNYIT with her MS in nursing she accepted a position
as an ANP at Planned Parenthood of Mohawk Valley. In 2002 she then
accepted a position with Dr Haider providing inpatient care in the
Acute Rehabilitation unit at Rome Memorial Hospital and outpatient
services for pain management at the Utica offices of APMR. Currently
she is providing out patient care to pain management clients at
our Utica and Clinton offices.
Debra is ANCC board certified in Adult Health and Pain Management Nursing.
Debra is considered to be an expert in the field of Pain Management and had
lecured at SUNYIT as well as the Annual Teaching Day for the Mohawk Valley
Nurse Practitioners Association.
Debra
is currently a member of the New York Nurses Association (NYSNA), American
Society for Pain Management Nursing, American Society of Interventional Pain
Physicians, and The New york State Nurse Practitioner's Association.
Sigma Theta Tau International Honor Society of Nursing, and The
New York State Nurse Practitioner's Association.

Pierre Herard, MD, FAAPMR, DABPM, FIPP
Dr. Herard brings a wealth of musculoskeletal experience to our practice. he received his medical degree from the National Autonomous University of Mexico in Mexico City in 1978. Thereafter, he completed a 4 year residency-training program in Orthopedic Surgery at the National Medical Center in Mexico City. In 1985, upon learning of an urgent need for an orthopedic surgeon in his wife's homeland of Jamacia, he moved to Jamacia, and worked in Kingston and later Montego Bay, where he was the sole orthopedic surgeon covering the western half of the island.
In January 1991, he came to the United States to pursue a visiting orthopedic fellowship at the Sinai hospital of Baltimore. Afterwords, he completed 1 year residency-training programs in Surgery at Brooklyn Hosptial Center, Anesthesia at Long Island Jewish Medical Center, and Internal Medicine at Long Island Jewish Medical Center.
In search of a specialty that could encompass all his interests, he entered the field of Physical Medicine and Rehabilitation in 1994. Dr. Herard completed a 3 year residency program in Physical Medicine and Rehabilitation at the Long Island Jewish Medical Center, where he also served and Chief Resident. He became a Board Certified Physiatrist in 1998.
Dr. Herard worked as an Attending Physiatrist and Clinical Instructor in PM&R for SUNY Downstate, and at the Bath Beach Medical Center, and affiliate of Staten Island University Hospital, from 1997 untill 2001. He also worked as an Attending Physiatrist as Staten Island University Hospital bewteen 1997 and 2004 and established a successful private practice with locations in Brooklyn and Queens, NY. The focus of his practice was musculoskeltal medicie and Pain Management.
Dr. Herard came to the Upsate New York area in 2004 with a strong desire to not only share his musculoskeletal experience, but also to enhance his pain-management skills. In 2005 he left for Minnesota to help set up a pain practice with a group of spine surgeons. Prior to rejoining our practise in 2008, he also worked with an anesthesiologist in Massachusetts in a pain practice. he has obtained certification in Pain Medicine asd is a Fellow of Interventional Pain Practice with the World Institute of Pain. He just recently recieved re-certification by the American Board of Physical Medicine and Rehabilitation. He lives in Clinton, NY with his wife Grace-anne who is a physical therapist.

John Minor, DO, FAAPMR
Dr. Minor completed his undergraduate in General Science and a B.S in Botany before receiving a M.S. degree in Biological Chemistry at the Hershey Medical Center of The Pennsylvania State University. He then went on to the medical school at the University of Medicine and Dentistry of New Jersey - School of Osteopathic Medicine where he graduated in 1987. He then completed an internship at the University of Pennsylvania - Presbyterian Medical Center in Philidelphia, PA.
Entering the field of Physical Medicine and Rehabilitation, he completed a Residency at New York Unuiversity in 1991. Following this, for more than a decade, he worked as an attending physiatrist for Kessler Institute for Rehabilitation in New Jersey, and institution consistently ranked within the top 10 such facilities in the country, during which time he was promoted to Senior Physiatrist and covered as acting Medical Director. There Dr. Minor had a busy practice providing both inpatient and outpatient services.
Dr. Minor is Board Certified in Rehabilitation Medicine and had publications in The Journal of Biological Chemistry and The Archives of Physical Medicine and Rehabilitation. Dr. Minor augmented his training by completing training in Medical Acupuncture through UCLA in 1997. He then moved to Ohio where worked as an attending in a multiple-city group practise beginning in 2006 where he saw outpatients with a variety of musculoskeletal conditions and provided inpatient coverage for a hospital general rehabilitation unit.
Finally, his interest shifting more to interventional pain management, Dr. Minor joined Dr. Haider's office in 2009. In addition to seeing patients in the office with a variety of neuromusculoskeletal and spine conditions, developing skills in fluoroscopically guided pain procedures, providing electrodiagnostic services, hospital pain management consultation services as St. Elizabeth's Hospital and St. Lukes Hospital, he regularly provides physiatric consultative services at Faxton's inpatient rehabilitation unit.
